  • You will update us promptly if any details change before your appointment.
  • Information submitted online supplements, but does not replace, in-person verification and clinical assessment at the hospital.
  • We may use the information you provide to prepare your file, contact your next of kin where appropriate, and deliver care in line with our clinical and legal obligations.

Submitting false or misleading information may delay care, affect registration, or constitute a breach of these Terms.
